Health Dept: Pandemic Flu Summit April 12, 2006, in Somerset, KY What: LCDHD Pandemic Flu Summit When: Wednesday, April 12, 2006, 9:30 a.m.-1:00 p.m. Where: Center for Rural Development, 2292 South Highway 27, Somerset , Kentucky 42501 Who: Open to public Sponsored By: The Lake Cumberland District Health Department, Serving: Adair, Casey, Clinton, Cumberland, Green, McCreary, Pulaski, Russell, Taylor, Wayne Lincoln and Garrard Counties Shelly Fryman Lake Cumberland District Health Department shelly.fryman@ky.gov The Pandemic Flu Summit will address the current Pandemic Influenza threat. Subjects to be covered include: the difference between a pandemic and epidemic disease outbreak, the steps individuals can take to protect themselves, community wide concerns, and resources that are available. Groups invited to attend: Lake Cumberland Civic Leaders, Daycare Providers, Faith Based organizations, Business and Community Leaders, First Responder agencies, Utility companies, Schools (public & private), Health professionals and the general public are invited to attend. Hors d'oeuvres will be served. Registration instructions follow: Following are instructions for registration. Please call Shelly Fryman (606) 678-4761 Extension 129 or any of the B-Team at (606) 678-4761 for any questions or concerns. To register, please go to the TRAIN website. The deadline for registration is Tuesday, April 3, 2006. So please do this as soon as you can. Following the conference, I will confirm attendance. You will then go to the TRAIN website to complete the evaluation tool. To complete the evaluation tool log onto your TRAIN account and click on the text "There are Pending Evaluations" above the My Learning Record. Click on the title of the evaluation that you wish to complete. After completion of the evaluation, click on the "Certificate Folder" in the "My Learning Record" and click the Certificate you wish to view/print.
