Free Sports Physicals Saturday, June 4, 2022

T.J. Samson Rehab and Sports Medicine will be providing free KHSAA-approved annual sports pre-participation physicals for student athletes in grades 5-12, from 7:30amCT TO 11amCT on Saturday, June 4, 2022, in the T.J. Health Pavilion Rehab Department, 310 N. L Rogers Wells Blvd., Glasgow, KY.

Note: This event provides sports physicals only.

Participants are required to wear a mask that will cover their nose and mouth.
Be sure to wear your corrective vision eye glasses or contacts.
KHSAA Pre-participation Physical Forms pages 1, 2, 4, 5 and 6 must be completely filled out by parent(s) or legal guardian(s) before arriving at the Sports Physicals event. Pages 3 and 7 will be completed at the event. All 7 pages of the Physical Forms must be brought to the event.
Parent or legal guardian signature is required.
You can print a copy of the form(s) from the KHSAA website:

Go to khsaa.org. Click on KHSAA forms at the bottom left of the home page in the black area.

High School Students: Click on General Administrative Forms (Physical, Transfer, Contract). Then click on GE04-Combined Parent Permission and Athletic Physical Forms (PDF) (High School). Print & fill out completely.

Middle School Students: Click on Middle School Forms, then click on MS01-Combined Parent Permission and Athletic Physical Forms (PDF) (Middle School). Print & fill out completely.
If the student plays on middle AND high school teams, then page 1 from GE04 and MS01 has to be filled out.
