Free Sports Physicals Saturday, June 4, 2022

T.J. Samson Rehab and Sports Medicine will be providing free KHSAA-approved annual sports pre-participation physicals for student athletes in grades 5-12, from 7:30amCT TO 11amCT on Saturday, June 4, 2022, in the T.J. Health Pavilion Rehab Department, 310 N. L Rogers Wells Blvd., Glasgow, KY.

Note: This event provides sports physicals only.
  • Participants are required to wear a mask that will cover their nose and mouth.
  • Be sure to wear your corrective vision eye glasses or contacts.
  • KHSAA Pre-participation Physical Forms pages 1, 2, 4, 5 and 6 must be completely filled out by parent(s) or legal guardian(s) before arriving at the Sports Physicals event. Pages 3 and 7 will be completed at the event. All 7 pages of the Physical Forms must be brought to the event.
  • Parent or legal guardian signature is required.
  • You can print a copy of the form(s) from the KHSAA website:
