John Maxwell Forbis, age 62 of Summer Shade, died Tuesday September 15, 2015 at the T J Samson hospital in Glasgow of natural causes.

Funeral services will be Friday, Sept. 18, 2015 at 10amCT at the McMurtrey Funeral Home, 2232 Summer Shade Road, Summer Shade, KY, with Bro. Mark Parks officiating. Burial will follow in the White cemetery in Sulphur Lick. Visitation will be Thursday, September 17, 2015 from 4pmCT until 7pmCT, and on Friday from 7amCT until the funeral at 10amCT. In lieu of flowers, donations may be made to the Barren River Animal Welfare Association, P.O. box 171; Glasgow, Ky. 42142

He is survived by the following:
  • One brother: Dr. Frank (Marilyn) Forbis of Hopkinsville, KY

  • One niece: Amy Lynn(Jason) Wilson

  • Two great nieces: Jacey and Jamie Wilson and cousins Roger, Jason and Tommy Forbis
In addition to his parents he was preceded in death by one brother, Henry Forbis, and a cousin Kenny Forbis.
