Parent Member Nomination Form

As you are probably aware most schools have a Site Based Decision Making Council that consists of members of the school community. The role of the council is to help in making decision in many aspects of the school. The SBDM council is made up of two parents, three teachers, and a principal.

Each year the council elects new members for both teacher and parent members. We are asking for your help in nominating parents to serve a one year term on the ACMS SBDM Council. Parents who serve must have a child attending Adair County Middle School during the 2019-20 school year.

Nominations will be taken Monday, April 22, 2019 through Friday, April 26, 2019, at 3pmCT. Nomination forms need to be turned in or faxed to Adair County Middle School, Attention Arrows PTO. They may also be emailed to arrowspto@gmail.com.

The election will then take place on Friday, May 10, 2019, from 7amCT to 3pmCT at Adair County Middle School, 322 General John Adair Dr, Columbia, KY. The election will be conducted by the PTO.
